Novel therapies entering overlooked disease categories often encounter steep adoption barriers when conditions remain underdiagnosed and unfamiliar to prescribers. Overcoming this resistance requires securing a strategic beachhead: a targeted focus area used to establish initial adoption and secure market traction, such as building diagnostic pathways, educating specialists on disease recognition, or establishing outcomes-based agreements with payers.

This case study is part of Triangle Insights Group’s series, Levers for Launch: Navigating Early Market Resistance Across Brand Archetypes.

The Commercial Dilemma

Pfizer launched Vyndaqel and Vyndamax (tafamidis) as the first FDA-approved treatment for transthyretin amyloid cardiomyopathy (ATTR-CM) in 2019. The therapy transformed a once-overlooked and underdiagnosed condition into a well-defined, treatable disease area, offering the first therapy proven to reduce mortality and cardiovascular hospitalizations in ATTR-CM patients.

The asset fits the Paradigm Shaper archetype, defined by high product differentiation within a nascent market.

Three main factors limited the early adoption of Vyndamax:

1. Underdiagnosis and Diagnostic Hurdles: ATTR-CM was largely an invisible and underdiagnosed condition, requiring the establishment of non-invasive imaging pathways to systematically identify patients.

2. Premium Launch Pricing and Payer Scrutiny: A launch price of $225,000 per year sparked market scrutiny describing the therapy as the world’s most expensive cardiovascular medication, prompting payer resistance and affordability debates.

3. Low Physician Awareness: General cardiologists lacked familiarity with the disease, requiring targeted education to recognize clinical red flag symptoms.

Activating the Four Levers to Build the Market and Secure Growth

By pairing evidence with physician education, Vyndamax established itself as the standard of care for ATTR-CM, driving broader adoption, shaping diagnostic algorithms, and sustaining growth:

Positioning Strategy: Pfizer urged physicians to find and treat ATTR-CM rapidly, pairing product positioning with campaigns teaching cardiologists how to suspect and diagnose the condition using a red flag symptom framework, before shifting to real-world outcomes and earlier intervention.

Advocacy and Guideline Inclusion: Leading academic cardiologists served as early adopters alongside patient advocacy organizations like the Amyloidosis Research Consortium, helping secure a Class I recommendation in the 2022 AHA/ACC/HFSA guidelines and 2023 ACC Expert Consensus endorsements.

Evidence Generation: Market entry was supported by the pivotal Phase III ATTR-ACT trial demonstrating a 30% reduction in mortality and a 32% reduction in hospitalization rates, reinforced by real-world registries, observational cohorts, and long-term survival analyses.

Market Access: Pfizer initiated value-based contracts tying rebates to real-world outcomes with PBMs, worked directly with payers to streamline prior authorization processes with clinical toolkits, and expanded copay assistance programs to address patient affordability concerns.

Key Strategic Considerations for Launch Teams

The commercial turnaround of Vyndamax highlights five considerations for commercial, medical, and market access leaders planning brand launches:

1. Build the Disease and the Drug. Vyndamax’s success hinged on transforming ATTR-CM from an invisible condition into a recognized, diagnosable disease. Future therapies should invest early in diagnostic infrastructure, physician education, and patient awareness so that the market exists when the product launches.

2. Anchor Pricing to Clinical Reality. The initial payer pushback against Vyndamax’s list price highlighted the need to align launch pricing with demonstrable outcomes and cost-effectiveness data to accelerate coverage decisions and minimize early access friction.

3. Leverage Advocacy as a Force Multiplier. Pfizer’s partnerships with KOLs, patient groups, and cardiology societies likely had the biggest impact of all activities in legitimizing ATTR-CM, shaping guidelines, and challenging payer restrictions thus demonstrating that advocacy engagement must be central to the launch strategy.

4. Expand Evidence to Sustain Leadership. Vyndamax defended its market position by continuously generating real-world data, earlier intervention studies, and registry evidence, ensuring clinical relevance to win over naysayers from the clinical and payer stakeholders even as competitors advanced RNAi and gene-editing therapies.

5. Evolve the Positioning and Narrative by Era. As the ATTR-CM market matured, Pfizer shifted from a “first and only” positioning to one focused on long-term outcomes, quality-of-life impact, and real-world validation therefore securing sustained leadership.
