Novel therapies entering previously untreated spaces often face steep adoption barriers when safety concerns or prescriber inertia arise. Overcoming this resistance requires establishing a strategic beachhead: a targeted focus area used to secure market traction, such as activating independent patient advocacy champions or publishing real-world surveillance data to resolve clinical skepticism.
This case study is part of Triangle Insights Group’s series, Levers for Launch: Navigating Early Market Resistance Across Brand Archetypes.
The Commercial Dilemma
Nuplazid (pimavanserin) launched in 2016 as the first and only FDA-approved therapy for Parkinson’s disease psychosis (PDP). It was positioned as a breakthrough for addressing hallucinations and delusions in Parkinson’s patients without worsening motor symptoms, aiming to displace widespread off-label use of atypical antipsychotics.
The asset fits the First Mover archetype, defined by low product differentiation within a nascent market.
Despite strong clinical need, three factors tempered the early adoption of this therapy:
- Safety Scrutiny and Media Backlash: High-profile media backlash and FDA adverse event reporting pointing to deaths in the clinical trial created market uncertainties regarding mortality risk.
- Physician Inertia and Entrenched Off-Label Care: The standard of care at launch was off-label antipsychotics. Clinicians required education on the burden of psychosis within Parkinson’s disease to transition away from familiar, suboptimal treatments.
- Payer Restrictions: Nuplazid faced early access hurdles given a list price of approximately $24,000 per year, prompting prior authorization requirements across commercial and Medicare plans.
Activating the Four Levers to Stabilize Trajectory and Secure Growth
Through transparent communication, intensive advocacy, targeted evidence generation, and robust patient access programs, the brand stabilized its trajectory and established standard-of-care status in PDP:
Positioning Strategy: Promotional messaging initially focused on Nuplazid as the first and only FDA-approved therapy for an indication with high unmet need. During safety scrutiny, messaging pivoted to a defensive, transparent strategy emphasizing that known risks are outweighed by benefits for appropriate on-label patients. Later campaigns framed Nuplazid as an integrated part of care that delays nursing home placement, expanding to patient subsets with or without dementia.
Advocacy and Guideline Inclusion: Key opinion leaders partnered with advocacy organizations including the American Parkinson Disease Association, Parkinson’s Foundation, and Michael J. Fox Foundation to drive disease awareness and amplify FDA conclusions that no new safety risks were discovered. In 2019, the International Parkinson and Movement Disorder Society and American Geriatrics Society updated treatment guidelines to specifically endorse pimavanserin.
Evidence Generation: Post-marketing safety surveillance and FDA analyses demonstrated that advanced patient frailty explained reported mortality. This foundation was supplemented by a large Medicare claims analysis showing reduced long-term care admissions, 2023 data showing lower mortality risk compared to other antipsychotics, and a 2024 head-to-head study demonstrating clinical benefit over quetiapine.
Market Access: Acadia launched the Acadia Connect hub to offer benefits verification, prior authorization assistance, and 14-day bridge supplies. Patient support programs expanded to provide a $0 copay for commercial patients and ensure 90% or more of patients paid under $10, maintaining broad coverage across Part D and commercial formularies.
Key Strategic Considerations for Launch Teams
The commercial turnaround of Nuplazid highlights five considerations for commercial, medical, and market access leaders planning brand launches:
- Create Conversation Early on the Need to Treat. Define the exact patient profile that exhibits clear unmet need and a need to treat from the start. This helps overcome entrenched off-label habits and accelerates adoption of a new standard.
- Enlist Champions and Build Credibility. Activate trusted KOLs and advocacy groups to validate the benefit-risk and carry the message during moments of skepticism. Independent voices stabilize confidence more effectively than company-led promotion.
- Data is Your Defense (and Offense). Continually generate and share evidence (from RWE to head-to-head comparisons) to defend against scrutiny and reinforce value. Transparent data use transforms potential threats into opportunities for differentiation and credibility.
- Access is as Critical as Efficacy. Premium-priced therapies must be paired with intensive payer engagement and robust patient support to remove financial friction. If cost blocks treatment, clinical innovation alone will not secure uptake.
- Adapt and Evolve the Launch. Each phase of a product’s lifecycle demands recalibration of levers from safety-focused messaging to refined positioning in new subgroups. Treat launch as a multi-year process that requires flexibility to sustain momentum.
